North Cynon Cluster currently has around 31,788 patients served by three major GP practices working from 5 sites
The cluster covers a large geographical area including Hirwaun Penywaun, Trecynon ,Cwmdare Aberdare Aberaman Abercwmboi and all surrounding areas.
There are high areas of social deprivation long term disability/ morbidity and chronic illness

The North Cynon Cluster is currently in the process of developing its plan for 2026 to 2027. This is based on the local population needs assessment and the emerging priorities identified by members of the cluster.
What we are working on
The cluster is developing projects to support the health and wellbeing of the local population. We have recently completed a Stop Smoking Project which was run in line with public health Wales
What we have already done
North Cynon has a Cluster Pharmacist, FCP Physio, Qualified Care Navigators, Vitality Therapies, Young Persons Counselling, Hollistic Therapist, Mental Health Practitioners, Wellbeing Coordinators which has enabled GPs to focus on patients with complex medical needs by navigating patients to different services .
North Cynon Cluster have employed an Advanced Nurses Practitioner to carry out the enhanced care home service this has been fundamental in supporting patients to remain well at home.
Future priorities for the cluster
The cluster is also working on new initiatives to address emerging priorities.
A specific priority for the coming year is developing ways to try and improve patent continuity with their GP or other clinician.
